The billing calendar. A program billing every 4 weeks charges 13.04 times a year. A “$249/month” on that cycle is really $270.62 per true month — $3,247 a year, not $2,988. Membership stacking. The cleanest example in this table: a flat $199 medication fee with a roughly $79 membership billed beside it, for $278 all-in. Neither number is dishonest on its own; comparing the $199 against another program’s all-in figure is.

The dose ladder. Several programs price 2.5–5 mg aggressively and step up sharply at 7.5 mg and above, while flat-at-every-dose programs (NexLife, bmiMD, HealthRX, Willow, Mochi’s medication fee) cost the same whether you titrate or not. If you expect a long titration, the flat structures usually win by month six even from a higher start.

The savings theater. Roundups love “save $937/month vs Zepbound,” computed against the ~$1,086 list price. Almost nobody paying cash pays list: Lilly sells Zepbound single-dose vials direct to self-pay patients for roughly $299–$449/month depending on dose [verify current LillyDirect pricing]. What changed on January 1 — and what most roundups still have wrong

Through 2025, Medi-Cal covered GLP-1 medications for weight loss with prior authorization, and several comparison sites still say it does. California’s 2025 Budget Act eliminated that coverage for weight-loss indications effective January 1, 2026; coverage for qualifying type 2 diabetes (Mounjaro, Ozempic under their diabetes indications) continues with prior authorization. Verify with Medi-Cal before relying on this — but do not rely on pages last updated before the budget passed.

Compounded tirzepatide itself is cash-pay: not covered by Medi-Cal, Medicare, or most commercial plans. Brand-name Zepbound is generally FSA/HSA-eligible with a prescription; some administrators accept compounded GLP-1s with a Letter of Medical Necessity — ask yours, in writing.

The FDA declared the tirzepatide shortage resolved in December 2024 and the semaglutide shortage resolved February 21, 2025, ending the shortage-era pathway for broad compounding; 503B outsourcing facilities had to wind down tirzepatide by March 19, 2025. Safety, briefly. Tirzepatide carries a boxed warning (thyroid C-cell tumors in rodents; contraindicated with personal/family history of medullary thyroid carcinoma or MEN 2).
